TakoTsubo Syndrome: A Well-Known Disease but Not Everything Is Clear Yet

TakoTsubo Syndrome (TTS) is a stress-induced cardiac disease characterized by temporary and segmental left ventricle dysfunction, typically involving the apex. Post-menopause women are more frequently affected. ECG clinical features at presentation may be similar to those observed in acute coronary syndrome (ACS). However underlying pathomechanisms completely different and, for what concerns TTS, extremely debated not yet understood. Some hypotheses have been proposed during years, mostly regarding catecholamine-induced cardiotoxicity microvascular usually following trigger event which either “emotional” (primary TTS) or “physical” (secondary TTS). Additional modulators like neuroendocrine disorders (particularly hypothalamic-pituitary-adrenal axis dysfunction estrogen drop menopause) play crucial role TTS onset. Despite being originally considered benign than ACS, several studies enlightened that STEMI burdened same in-hospital mortality complications. However, ACS complications somehow differ incidence, mechanisms, both long- short-term outcomes. Full recovery requires weeks months cases of recurrences described, but no single feature seems predict subsequent episodes so far. By now, apart from inhibitors Renin-Angiotensin-Aldosterone System (RAASi), drug has proved effective chronic phase reducing mortality, improving outcome, preventing recurrences.
